If you are uninsured or underinsured, you may qualify for our Sliding Fee Scale — visit cost adjusts to your household income and family size. No one is turned away for inability to pay.

- Based on household size and annual income — documentation kept confidential
- Applies to primary care, ophthalmology, behavioral health, and lab services
- Re-evaluated annually — bring your most recent tax return or pay stubs
- 100% FPL or below may qualify for nominal or zero-cost visits
